Medical Forms
BOTH OF THE FOLLOWING ITEMS ARE REQUIRED TO PARTICIPATE IN ATHLETICS AT SUNY NEW PALTZ

1. MEDICAL FORMS COMPLETED THROUGH THE INSTRUCTIONS BELOW

  1. All forms will be completed through a system called ARMS. Here is the link:
    1. https://sso.armssoftware.com/ (Your coach will give you instructions as to how to login to this system)
  2. A number of medical forms will be assigned to all student-athletes in early June with an expected completion of July 15th.
  3. If you have questions, please contact HawksAT@newpaltz.edu          

2. A UP-TO-DATE PHYSICAL COMPLETED BY YOUR PHYSICIAN USING THE HEALTH REPORT BELOW

  1. A Health Report needs to be completed by all new student-athletes which include freshmen, transfers, and anyone who did not compete in athletics the previous season.
    1. Most returning athletes have already completed this step.
  2. You should receive this by email from Student Health Services.  If you did not, here is a link to the Health Report. 
    1. It is recommended to only use this form and NOT the form provided by your doctor's office. 

  3. Upload immunization records and medical documents using the Medicat Patient Portal.  

    The Portal can be accessed: 

    - New Student Info: Take Flight 
    - The Student Health Service Website 
    - https://newpaltz.medicatconnect.com 

    This needs to be completed and returned by the deadline for your team's medical paperwork or August 1st (whichever is earlier)
